Can’t visualise or hit the vein?  

  • How cardiovascularly stable is the patient? 
  • Change position to aid in visualisation 
  • Consider the use of ultrasound to guide placement 
  • Ensure the patient is not hypothermic 

 

Can’t feed the guidewire?  

  • Check you have blood flow from the catheter  
  • Turn the guidewire around so it is no longer a J end – take care not to cause trauma to the vein. 

 

Lost the guidewire?  

  • NEVER let go! If it gets sucked in during placement the patient will require imaging to locate it, and then retrieval. 

 

Can’t feed the catheter over the guidewire?  

  • Check the guidewire is still in the vein  
  • Make a larger skin incision at the insertion point  
  • Dilate the subcutaneous space again
